I switched prescribers last month. The handover was easy because I had a year of notes.

When I changed clinic — different platform, different prescriber, different system — the bit I was dreading was repeating my history from memory. What I'd been on, when, what I'd noticed, when things had changed. I didn't have to.

I'd been writing it down in Titra all along. Pulled up the timeline at the first appointment, walked through it, took about four minutes. The new prescriber said it was the most useful handover she'd had that week.

Titra didn't do anything clever — I just had a year of my own notes ready. The app stays on my device. The notes are mine.
